New Biomarker Outperforms Standard Tests in Critical Kidney Care

A study of 1,436 critically ill patients reveals that the biomarker proenkephalin A 119–159, or penKid, offers a more precise method for managing kidney replacement therapy. Published in the Annals of Intensive Care, the findings suggest penKid can identify patients at high risk of treatment failure better than traditional diagnostic tools.

Researchers at Heidelberg University Hospital tracked penKid levels to address the limitations of serum creatinine and urine output, which often fail to provide a complete picture of renal health in the ICU. The data demonstrated that penKid levels rise in direct correlation with acute kidney injury severity. Notably, the marker successfully distinguished between stage 3 patients requiring intervention and those with pre-existing chronic conditions, even while therapy was ongoing.

The study highlights a significant advantage for clinical decision-making: predicting liberation failure. While serum creatinine levels often drop during treatment regardless of underlying recovery, penKid levels decline only upon successful liberation. A threshold of ≥250 pmol/L demonstrated over 90% specificity in predicting patients who would fail to transition off kidney replacement therapy. This objective parameter provides clinicians with a clearer signal to avoid premature discontinuation of support, potentially standardizing care protocols in high-stakes intensive care environments.
