Illinois Expands Medicaid Coverage for Preterm Birth Blood Testing

Illinois has amended its Public Aid Code to mandate Medicaid coverage for proteomic blood testing designed to predict spontaneous preterm births. This legislative shift aims to curb the state’s high rate of premature deliveries, which saw over 13,000 instances in 2024, by facilitating earlier clinical intervention for high-risk patients.

The new mandate requires the state's medical assistance program to reimburse both the diagnostic testing and the subsequent care management services necessary to improve neonatal health outcomes. Sera Prognostics, the manufacturer of the PreTRM test—the only commercially available proteomic blood test for this purpose—confirmed the development, noting that the initiative aligns with a broader push to prioritize evidence-based diagnostics in maternal care.

With approximately 50,000 births funded by Medicaid in Illinois annually, the state is looking to leverage risk stratification to manage both clinical complications and the significant financial burden associated with prematurity. While the specific timeline and coverage parameters remain under development, the company is actively collaborating with healthcare stakeholders to define the implementation framework. By identifying risk between the 18th and 20th weeks of pregnancy, the policy seeks to shift the standard of care from reactive measures to proactive, personalized intervention.
