The group aims to support over 100,000 individuals across eight states using a unified, AI-integrated medical records system.

The Oregon Wellness Network (OWN) leads a coalition of eight hubs spanning from Alabama to New York, designed to keep older adults and people with disabilities independent in their homes. By focusing on care transitions, fall prevention, and nutritional support, the consortium seeks to lower hospital readmission rates and reduce overall healthcare spending. The initiative marks a significant shift in community-based medicine, utilizing a shared AthenaOne electronic health record to synchronize care delivery across diverse geographic markets.

For Oregon residents, the victory scales an existing infrastructure that already connects local Area Agencies on Aging with major providers like the Providence health system. The program introduces five specific care pathways, ranging from post-hospital support to clinical interventions for social isolation. Lavinia Goto, Director of the Oregon Wellness Network, emphasized that the collaboration represents a new national capability, proving that community-based organizations can effectively function as a single, tech-enabled network. The team will now move into an implementation phase lasting through July 2027, with the potential to advance to a final scaling round next summer.
